- COVID-19 감수성과 ACE2 효소 간 인체 계통별 영향요인 분석 : 체계적 문헌고찰 -

본 연구는 감염 발생에 따른 COVID-19 감수성과 ACE2 효소 간 인체 계통별 영향요 인을 탐색하였다. 연구방법은 체계적인 문헌고찰 분석 방법으로 2021년 12월 30일부 터 검색 데이터 베이스를 이용하여 키워드 ‘COVID-19’, ‘Spike protein’, ‘ACE2 (Angiotensin - Converting Enzyme 2, ACE2)’, ‘susceptibility’에 해당하는 핵심단어 MeSH 용어와 text word를 온라인 검색 시스템을 통해 검색하였다. 연구 결과 COVID-19 감수성이 호흡기계, 피부계, 순환기계, 내분비계, 신경계, 비뇨기계의 계통 순에서 영향을 미치는 것으로 나타났다. 그중 호흡기계에 가장 많은 영향을 미치는 것 으로 분석되었다. 주요어 기반 요인별 특성 분석에서는 유전적 요인, 환경적 요인, 바이러스 특성 관련 요인, 기타요인으로 분석되었다. 결론적으로 COVID-19 감수성과 ACE2 분포 간 상관관계가 질병확산에 관련 있는 것으로 나타나 감수성과 상관관계를 중심으로 한 추후연구가 요구되며, 무엇보다 예방을 중심으로 한 변이바이러스 특성에 맞는 백신 개발과 예방접종이 질병 극복에 도움이 될 것으로 사료된다.

Analysis of Factors Affecting Human body Systems in Relation to COVID-19 Susceptibility and ACE2 Expression : A Systematic Review

This study investigated factors affecting COVID-19 susceptibility and ACE2 expression according to infection incidence by human body system. The study method used was a meta analysis by conducting a systematic review of the literature using database search from December 30, 2021. MeSH terms and text-words related to keywords including ‘COVID-19’, ‘spike protein’, ‘angiotensin-converting enzyme 2 (ACE2)’, and ‘susceptibility’ were searched on online search engines. An additional study was performed from January 20 to Feburary 15, 2022 by combining terms using the most commonly used Boolean operators with search formulas. The study findings showed that susceptibility to COVID-19 infection increased in the order of respiratory, integumentary, circulatory, endocrine, nervous, and urinary system. Of these, the respiratory system was most susceptible to COVID-19. The analysis revealed that keyword-based characteristic factors were genetic, environmental, virus-related and other factors. To conclude, a correlation between COVID-19 susceptibility and distribution of ACE2 expression were found to be related to infection spread. For this reason, further studies on this correlation are warranted. Moreover, development of effective vaccines against variants of concern and prophylactic immunization focused on prevention are thought to essential for overcoming COVID-19.
